PATIENT BILLING & FEES

Horizon Anesthesia offers you a simple payment structure.

Horizon Anesthesia requires a minimum deposit prior to the date of your procedure. This deposit will be collected by your physician/dentist office on our behalf and covers the cost of medication, equipment and your anesthesiologist’s professional charge for the first hour of your care. We accept credit cards or cash.

On the day of your anesthetic you will be asked to pay (again by credit card or cash) additional anesthesia time above your initial deposit. Your anesthetic time is defined as the point in time when the anesthetic is administered to the time of your transfer to the recovery area. Generally anesthesia time will be longer than the procedure time.

We are happy to assist you in obtaining the maximum reimbursement allowed by your insurance carrier, if applicable. Horizon Anesthesia will mail an insurance reimbursement form to you or your designee within 30 days of the anesthetic. This form will describe the exact nature of the anesthetic we performed, the reasons for the services we provided, and the appropriate CPT code (billing code). This form is then submitted by you to the insurance carrier.
